What is a Care Plan?
A care plan is a plan prescribed by your doctor, when you meet certain medical criteria. This entitles you to five visits claimable through Medicare each calendar year.

What is your cancellation policy?
We strive to provide an excellent and specialised service, with adequate availability that suits your timetable. Certain appointment times, particularly after-hours appointments are highly sought after. We do our best to ensure you won’t miss your valuable appointments by offering a free reminder service. We expect however that you respect that late cancellation of appointments or non-attendance disadvantages not only your treating practitioner, but other patients who may have been in great need of an appointment. Failure to provide as least 1 full working days’ notice of cancellation will attract a cancellation fee of $50 of your treatment charge. If you fail to attend your appointment, the full treatment fee will be charged.
It is not the aim to make profits from cancellation fees, rather to make sure those appointment times are kept available for others who need them. Failure to attend an appointment is unfair to other patients and take up the dietitian’s time unnecessarily.
Should you need to change your appointment, please contact the clinic as least 24 hours prior to the scheduled time to avoid paying the full fee. Chronic Disease Management (formerly EPC) will be billed our private consultation fee.
Clinic-based Private Consultations
Consultations are covered under ‘extras’ in your private health fund. Check with your private health insurance to see if you are eligible. Refunds vary depending upon your private health insurance. Fees are payable at the time of consultation and Cash, EFTPOS, Visa and MasterCard are accepted. Long consultations by arrangement.

Can I get a Medicare rebate?
You are able to claim a Medicare rebate towards the cost of your nutrition consultations if you are referred to Adept Nutrition Care by your GP on a Chronic Disease Management Plan.
Chronic Disease Management plans are designed to help patients to access the services of a range of health providers to help them manage their chronic (ongoing) health conditions.
CHRONIC MEDICAL CONDITIONS AND COMPLEX CARE NEEDS
A chronic medical condition is one that has been or is likely to be present for at least six months, e.g.: asthma, cancer, cardiovascular illness, high blood pressure, high cholesterol, stroke, diabetes mellitus, obesity, malnutrition, irritable bowel syndrome, musculoskeletal conditions and others.

HOW DO I GET A REFERRAL FOR A MEDICARE REBATE?
The only way to claim a Medicare rebate for your nutrition consultations with Adept Nutrition Care is to get a referral from your GP. Make a time to discuss this with your GP and/or the practice nurse. To make things easier, print the referral request letter below and take it to your GP.
HOW DO I CLAIM MY MEDICARE REBATE?
If you are referred to Adept Nutrition Care on a Chronic Disease Management Plan you will have to pay the full consultation fee at the time of your consultation then I process the Medicare rebate for you. Medicare rebate will then be deposited into your nominated bank account.

HOW MUCH IS THE MEDICARE REBATE?
A $25 out-of-pocket fee for an initial consultation with a $53.80 Medicare rebate and $10 out-of-pocket for follow up consultations. Fees are payable at the time of consultation and Cash, EFTPOS, Visa and Mastercard are accepted.
